Micrograms, milligrams and units
The arithmetic below uses an arbitrary dose to show the steps. Note the unit switch — it is where thousand-fold errors happen:
1. Concentration
Vial amount ÷ water added. A 5 mg vial with 2 mL of water is 2.5 mg/mL.
2. Volume per dose
Your dose ÷ concentration. A 0.2 mg dose at 2.5 mg/mL is 0.08 mL.
3. Units to draw
Volume per dose × 100 for a U-100 syringe (or × 40 for a U-40). 0.08 mL on a U-100 syringe is 8 units.
Worked example
You have a 5 mg vial, you want a 200 mcg dose, and you add 2 mL of bacteriostatic water. Concentration is 5 ÷ 2 = 2.5 mg/mL. Volume per dose is 0.2 ÷ 2.5 = 0.08 mL. On a U-100 syringe that's 0.08 × 100 = 8 units, and the vial holds 25 doses.
The mg/mcg trap
Compounds dosed in micrograms out of milligram vials are where the single most consequential arithmetic error in this whole process occurs: entering a microgram figure into a milligram field, or the reverse. The difference is a factor of one thousand, and unlike most mistakes it produces a plausible-looking number rather than an obviously wrong one.
The calculator's unit toggle exists specifically for this. Set the unit before typing the number, and read the resulting units back against what a full syringe holds — if the answer is a fraction of a unit or several barrels' worth, the unit is wrong rather than the arithmetic.
